This student absence form is designed to help teachers and students keep accurate and up-to-date records for when a student is absent. This form helps ensure that students do not miss out on important learning opportunities or miss important deadlines. The form is simple to complete, requiring the student’s name, the date of their absence, and what was covered in each subject area. 

Use this form in the classroom when a student is absent. The teacher or another student can fill this form out for the student that is absent. You can list what was covered in class for each subject area and also any homework that was assigned. My students and I have found this a very useful tool for keeping organized. For the student, there is no question as to what they missed while they were out.

Use Suggestion:

Print one or all of the styles provided

Keep the printed copies in an easily accessible part of the classroom

Possibly assign a student each day or have it as a classroom job to be the one to fill it out for students who are absent.

Place the filled out form and all other worksheets etc needed from that day on the absent student’s desk for when he/she returns.
