This student self-reflection form is offered in PDF and Google Forms versions. 


Students will answer questions and rate themselves on a scale of 1-5 and explain why they rated themselves how they did on the following:

✔ Their understanding of content

✔ Their study skills and habits


I used this reflection form to better understand where my 6th grade students were in terms of their understanding of content and study skills. It also doubled as a great talking point at parent-teacher conferences because parents were able to see where their child felt they were at in their learning in their own words and it helped us come up with a plan of action to better support their child throughout the year.
